If you are a member of Ostomy Tasmania and would like to place an online order for your appliances, please complete the following form and press the “submit” button.
If required, please ensure you have enough credit to cover postage (NB: packing slips sent with parcels will show the latest balance on your account. Parcels over 6kgs will be charged extra). If you intend to make payment at the time of ordering, or you are wanting to order 2 month supply (you need to have been a member for 6 months), please advise via the “special instruction” field in the form.
If you have a 2nd stoma please submit in a separate order form (i.e. 1 order form for each stoma).
Postage payments can be made anytime via this link (any excess will be credited to your account). Click here for ordering guidelines.
FORM SUBMISSION: After submission a “Thank You” message confirming your order will appear in its place on the same webpage (you may need to scroll up the page to see this). If you do not see the message, or get an error message, please contact our office.

ORDER FORM
Important: Due to June Stocktake some July orders will take longer to despatch than usual. Orders from suppliers are placed weekly. Coloplast, Salts, Ainscorp, Omnigon & Convatec products are ordered Monday mornings (by 10am). Dansac, Hollister and Braun products Tuesday mornings. Orders received after close of business at the end of the month will be processed the following month. Orders received before the issue month will be despatched at the end of the first week of the issue month (assuming items need to be ordered in).

Donations assist us to maintain our service to ostomates. Many of our office workers are volunteers, thus helping to keep our costs to a minimum for members, however our expenses continue to rise over time, so any donation is gratefully received, even if it's only a few dollars. Just click on the link and enter your credit card details via the secure payment gateway (transaction receipt will follow). Remember donations over $2 are tax deductible. Donations left in your will are also most welcome.
